Experts say new HIV infections have fallen by seventeen percent since two thousand one. Estimates for sub-Saharan Africa are down by about fifteen percent. In East Asia new infections with the virus that causes AIDS have decreased almost twenty-five percent. In Eastern Europe, the epidemic has leveled off. But new infections appear to be rising again in some countries. The report came at the end of November from the UNAIDS program and the World Health Organization. It says HIV-related deaths appear to have reached their highest point in two thousand four. Since the peak, deaths have fallen by around ten percent as more people have received treatment. Experts credit the good news in the report at least in part to prevention programs, not just the natural progress of the epidemic. Yet the report points out that while the AIDS epidemic is changing, prevention programs are not. Karen Stanecki at UNAIDS says few programs, for example, are designed for people in secure relationships. Or people over twenty-five. Or the newly single. AIDS is the leading cause of death in women age fifteen to forty-four. Those are the main years for having children. It is believed these men are among the men who meet in a secretive and marginalized fashion in parks, restrooms, or other public places with the intention of having sex elsewhere or at the public location. It is hard to estimate the number of men in this population, but it is estimated there are over 300 such locations in metropolitan Bangkok alone (TREAT ASIA, 2006). Frequently, the male-to-male sex between these often masculine-identified MSM happens quickly and furtively due to the location. This lack of time often leads to unsafe sex without condoms (MAP, 2005). In Chiang Mai, Mplus+s outreach workers estimate there are well over 30 such public places and they have a regularly updated hotspot map. This animation will be used on mobile phones and other portable devices and Mplus+ to target these hotspots on their weekly outreach. Based on the interview data of a significant transgender population in Chiang Mai, Mplus+ decided to produce an animation that empowers transgender individuals to negotiate condom use with their partners. This animation was piloted with the target population and the feedback was positive. More importantly, as an animation that is made exclusively for transgenders, the narrative itself resonates with transgenders lifeworlds and provides a catalyst for informed discussion in Mplus+s outreach to this population that goes beyond simply advocating condom use. This animation is directed towards the diverse MSM population in Chiang Mai. Individuals in this group may identify as gay or bisexual and generally tend to be young. Thus, they missed out on the successful HIV campaigns of the 1990s and often have low perceptions about personal HIV risk. Recent reports suggest around 85% of Thai youth do not see HIV as something that they should be concerned about, even though 70% of all STI cases in Thailand occur among this group (IRINnews, 2006). This makes them an important population for Mplus+ to target with HIV prevention. Additional reports of sexually active young Thais show only 20-30% are using condoms consistently (UNAIDS, 2006). Public information on HIV prevention, which was once ubiquitous, has virtually disappeared and no mass public HIV prevention campaigns have been carried out for over a decade. Mplus+ recognises this and the serious decline in risk awareness among young MSM, which point to a likely increase in unsafe sexual behaviour. In Thailand, it is reported that HIV transmission is understood, but personal risk about sexual practices is not (TREAT ASIA, 2006). MSM seem to know about HIV prevention practices and tools (condoms and nonoil based lubricants), but still do not use them regularly and there is significant lack of knowledge regarding other STIs and prevention practices. Thai MSW and migrant MSW primarily from the Shan, Karen & Burmese regions will benefit from this animation available in Thai and the 3 regional dialects. There are no recent reports of HIV prevalence among MSW in Chiang Mai, but in Bangkok the prevalence is reported to be 15.4% (venue-based) and 22.6% (street-based) suggesting prevalence among Chiang Mai MSW could also be dangerously high. Mplus+ understands Thai and Migrant MSW work across a variety of venues including bars, saunas, massage parlours, karaoke lounges, and brothels. Additionally, many work independently on the streets, in parks or cinemas. Because of the many different kinds of MSW that work in these establishments for different reasons, most work in isolation and many do not fully identify themselves as sex workers. As a result, they have little access to specific sexual health education or services and are most often underrepresented in current programming and advocacy (TREAT ASIA, 2006). It is important to point out that HIV epidemics move from vulnerable groups to the general population when there are links between the two. The link between MSM, MSW and women in Thailand is well established.
