Alexa Kelly Aigner was born on Thursday, April 29, 2004. She was the first child and only daughter of Zack and Crys Aigner. She was the light of their life and they loved her so much. In October 2005, when Alexa was 18 months old, she had a small bump at the bottom outside corner of her left eye. It was not worrisome as it would depress when touched but it was not normal. Crys and Zack took Alexa to the pediatrician. He reffered them to an ophthalmolo​gist who reffered them to a plastic surgeon at Children's Medical Center of Dallas. On December 12, 2005 after an MRI, they met a doctor who told them that Alexa's bump was in fact caused by a tumor growing behind her eye and it had metastasize​d from a larger tumor growing on her left adrenal gland. Further testing revealed several other tumors, bone disease, bone marrow disease and a diagnosis of Stage 4 Neuroblastoma. Shortly after Alexa's 2nd birthday, Crys and Zack found out Alexa's cancer had not responded well to the treatment she was on and would need to search for other alternatives. With the help of their doctor, they chose to do an expiremental treatment at the Children's Hospital of Philidelphia. They did a treatment called MIBG-a liquid radiation sent through Alexa's entire body that was ment to find and kill those cancer cells-followed by a single bone marrow transplant. TITLE: Single Port Access (SPA) Proximal Gastric GIST Resection Background: Recent advances in minimally invasive surgery have included methods for reducing the number of incisions for trocar placement. Single Port Access is a novel method allowing the performance of standard laparoscopic procedures through one incision. We have applied Single Port Access (SPA) surgery to procedures of the gastroesophageal (GE) junction through a single skin incision, usually placed within the umbilicus. We report the first Single Port Access resection of a proximal 3cm x 3cm GIST tumor in a patient presenting with an upper gastrointestinal bleed. Methods: Entry into the abdomen was achieved through an 18-mm umbilical incision for placement of a 5-mm trocar for the laparoscope. Skin flaps were subsequently created lateral to the laparoscope for placement of 2 additional trocars for the dissecting instruments. Once the single-port access had been achieved, the dissection of the mass was carried out with the collaboration of the gastroenterologist who performed an upper endoscopy, aiding in the eversion and dissection of the mass and evaluating the gastric resection site intraluminally. The mass was removed successfully, and no intraoperative and postoperative complications occurred. Conclusion: This is our first GIST resection done via a combined NOTES/Single Port Access approach. A 7-year-old girl from Long Island, NY, is on her way home a little more than four weeks after receiving a historic surgery that involved the removal and partial re-implantation of six organs in order to resect an abdominal tumor that otherwise would be inoperable. The 23-hour surgery, which began on Feb. 6, was led by Dr. Tomoaki Kato [(212) 305-5101, 877-LIVER-MD] at NewYork-Presbyterian Morgan Stanley Children's Hospital, and is the first reported pediatric case of its kind. Dr. Kato and his team of seven surgeons and eight other clinicians removed nearly every major organ in the girl's abdominal cavity -- small and large intestines, liver, pancreas, spleen and stomach. Three separate surgical teams worked to remove the tennis ball-sized tumor, which was tangled around vital organs and essential blood vessels. One team worked on the abdominal cavity, while two others prepared the small and large intestines and liver, which were kept healthy outside the body using cold preservation fluid of the kind used in transplantation. Throughout the surgery, the patient's father, a firefighter, was on standby as a transplant donor in case his daughter's liver was too damaged; it was determined to be healthy and viable. The liver, small and large intestines were re-implanted. However, her pancreas, spleen and stomach -- non-vital organs that had been compromised by the tumor -- were unsuitable for re-implantation.
