During the late eighties, Prof. Ilan Timor, while working in Haifa, Israel, was one of the world's pioneers in transvaginal sonography. His slogan "Do it with higher frequency" was based on the fact that higher ultrasound frequency allows supreme resolution on imaging of the female internal genital organs and of the fetus in a very early stage. This high frequency is possible only by using transvaginal ultrasound probes. High frequency waves poorly penetrate the abdominal wall. If a probe is laid on the external abdominal wall, lower frequencies (and hence worse resolution) must be used in order to penetrate the barrier of the women's belly. Early screening for fetal malformations by transvaginal sonography (at 14-15 weeks of pregnancy when the crown-rump length of the fetus is about 8-9 cm) is common practice in Israel and most fetal anomalies can be detected at this early stage. Though it is done routinely in Israel in the last 15-20 years, somehow until now (2009) the rest of the world has not yet adopted it. In the last few yeas Ilan Timor is working in New-York University in Manhattan and is considered to be one of the world leaders of OB/GYN ultrasound. The Double Line is the renal capsule and the Peritoneum with perinephric adipose tissue inbetween. Basically you are visualizing degrotta's Fascia. The perinephric fat is recognized by its internal echos and location between the 2 hyperechoic tissue layers. This is analogous to seeing a pericardial fat pad around the heart on the FAST or echocardiographic exam. This is not free fluid but can commonly be mistaken for such especially if the person is rushing, does not follow protocol, or has improper gain settings.
