Me, undergoing Laparascopic Cholecystectomy Procedure at MMC on 9/01/2010. I couldn't upload the whole video thru youtube since it's more than 15 minutes, so i just snatched a few minutes of the procedure from a camera. well, i just want to thank God for keeping me safe the whole time. i'm still in pain though, but this will pass, at least those gallstones (unfortunately, including my gall bladder) are out. i will just have to keep watch of my diet from now on.
This is a standard 04 port key hole cholecystectomy performed in a 43 year old diabetic and hypertensive patient. This procedure is done as early cholecystectomy during acute attacj of cholecystitis. Due to oedema and wall thickness , gall bladder was emptied first using a percutaneous 18 G LP needle. After the cholecystectomy the gall bladder is extracted using an improvised pouch made from the rubber hand glove. The procedure took 45 minutes and the payient had an uneventful recovery

Single Incision Laparoscopic Cholecystectomy is done through a skin incision of around 2cms at umbilicus. Through this single incision, 3 trocars are placed but at different levels at sheath. Central trocar is of 10mm & two 5mm trocars are placed on each side. Central port is used for 10mm 30degree telescope so as to have best endovision. Grasping and retraction is done using both 5mm ports. Once cystic duct & artery are dissected, if 5mm clip applicator is not available then 5mm telescope is used through 5mm port & 10mm clip applicator is used to apply clips on artery & duct. Further dissection of bed is done using hook/harmonic scalpel, sticking to the principle of staying lateral to Glissons capsule. Thorough suction & irrigation is done before delivering the gall bladder through 10mm port. Single incision laparoscopic Cholecystectomy is feasible using standard laparoscopic instruments. However surgeons experience is the key factor for surgical outcome. Finally to know its safety, place & benefits in comparison to standard laparoscopic Cholecystectomy results of long term studies need to be seen.
