SILS Sigmoidectomy for Colon Cancer with ColonRing. Courtesy of: Friedrich Herbst (MD, FRCS), Bernhard Dauser (MD) Department of Surgery, St John of God Hospital, Vienna A 35 year-old female patient had undergone colonoscopic removal of a 2cm polyp at 20cm from the anal verge. Histopathologic examination demonstrated a pT1 cancer with free resection margin but lymphovascular invasion, therefore resection was planned. Preoperatively, the site of the tumour was tattooed endoscopically using India ink. The SILSTM Port is introduced through a 20mm long vertical incision through the umbilicus. Patient is tilted to the right, head down in Lloyd-Davies position to facilitate arrangement of the small bowel (not shown in the video). After mobilization of the left hemicolon lateral-to-medial the IMA (Inferior Mesenteric Artery) gradually is isolated by a medial approach without compromising the preaortic nerve plexus and divided 1-2cm from the aorta. The upper rectum is transected with a 60mm blue cartridge Endo-GIATM linear stapler. The bowel is extracted through the umbilical incision using a plastic wound protector after removal of the SILSTM Port. After resection of the sigmoid colon a tension-free end-to-end compression anastomosis is performed using the Compression Anastomosis Ring CAR 27 TM / ColonRing TM (by NiTi TM Surgical Solutions www.nitisurgical.com) The postoperative course was uneventful. A 46-year-old woman presented with a history of 3 days of pruritus in the anal area and 1 day of excretion of tapelike materials. During the year before presentation, she had reported intermittent colicky abdominal pain and loose stool, which had been attributed to irritable bowel syndrome. Laboratory evaluation was unremarkable, with no evidence of anemia. Colonoscopy revealed a long, moving tapeworm, Diphyllobothrium latum, located in the terminal ileum and extending to the sigmoid colon. D. latum is a fish tapeworm that can infect humans after they consume infected undercooked or raw fish. The patient had a history of eating raw fish and recalled eating raw trout most recently 2 months before presentation. She was treated with a single dose of praziquantel. After administration, the abdominal pain resolved, but she continued to have intermittent loose stool.

A proctoscope is a short (10in or 25 cm long), straight, rigid, hollow metal tube, and usually has a small light bulb mounted at the end. This clip demonstrates a normal colonoscopy and starts in the terminal ileum where the delicate villous architecture is well seen. The colonoscope is withdrawn into cecum. Here at the base of the cecum is the orifice of the appendix. In the tissue surrounding the appendiceal orifice, a speckled pattern of lymphoid aggregates can often be seen. During the colonoscopy, the endoscopist both insufflates the lumen of the colon with air and aspirates the small amount of liquid that remains from the preparation previously taken by the patient to cleanse the colon. The liquid often has a slightly yellowish tinge of bile. Backing out of the cecum, a better view of the ileocecal valve is obtained. It has a rounded slightly yellowish appearance. Traveling down the colon, a repeating pattern of pouches and folds are seen. These pouches are called haustra and the folds are referred to as the plicae semilunares or simply the semilunar folds. A word about the colonic tissue itself. The white dots seen throughout the exam are reflections of the colonoscope's light off the tissues and back into the viewing lens. Normally the tissue has this glistening pink hue. Also visible is the delicate vascular plexus of capillaries that nurture the colonic mucosa. Here in the sigmoid colon, the earliest signs of diverticulosis are visible. These are outpouchings of the mucosa through the muscular layers of the colon and are seen commonly starting in the fifth decade of life. More dramatic examples ...

Endometrial tissue goes on in descending order: ovaries, anterior and posterior cul-de-sac, posterior broad ligaments, uterosacral ligaments, uterus, oviducts, sigmoid colon and appendix, round ligament. Appendix in 2-4% cases. Has even been reported in breasts, spine, lungs, and extremities. Can cause adhesions.
