Terry Sedlacek, whose family said had suffered from Lyme disease for years, stood up during Sunday service and shot and killed pastor Fred Winters at the First Baptist Church. He wounded to others. There is no definite relationship between Lyme disease and mental illness.
Steve Smick walked into the well-known Crystal Cathedral, knelt down in front of a cross and shot himself in the head. When he walked in he had handed a note to a church greeter. Smick had had a long history of mental illness.

Dennis John Malloosseril was shot and killed at the St. Thomas Syrian Orthodox Knanaya Church when he tried to break up a fight between Joseph Pallipurath and his estranged wife Reshma James. Pallipurath had driven from California to the church to confront her. He also killed her.

An unemployed truck driver, Jim D. Adkisson, killed two people after opening fire at the Tennessee Valley Unitarian Universalist Church. He pulled a gun out of a guitar case and started shooting during a children's performance of the musical "Annie." Adkisson had left a note that suggested that he targeted the congregation because of his hatred for its liberal policies and acceptance of gays.
A man shot and killed two church members at the New Life Church right before Sunday services ended. A female security guard shot and killed the gunman after he wounded two more people.
